5- to 11-years-old:
To schedule the first dose of your child's vaccination, please select a time for the desired date and location.
Patients 5- to 11-years-old must be accompanied by a parent or legal guardian to receive the COVID-19 vaccination.
If you do not see a date and time that works for you, please try back at a later time. We will continue to post new appointment times depending on vaccine supply.
